The Safety Sleeve program is designed to help our paramedics find valuable medical information about you when you are unable to speak, or are unconscious. The sleeve has a magnet on the back and should be placed on your refrigerator to make it easy for our paramedics to find this information. Inside of each sleeve is a sheet containing important information such as your name, date of birth, medical history, medications, allergies, etc. You can place more than one sheet in the sleeve if more than one person lives in your household. Please fill out each sheet as completely as possible, and keep the sheet updated as you change medications, incur a new medical diagnosis, etc. We also ask that if you have a valid Do Not Resuscitate Order that you keep a copy in the sleeve as well so that we can honor your wishes.
Sleeves and/or additional sheets can be obtained from our personnel who respond to your house for an incident, or can be obtained M-F 8A-5P at our Fire Headquarters located at 375 Nilles Road. You can also download and print the medical information sheet by clicking the link below. It is a fillable PDF form that you can type the information in, save it, and then print it off and place it in the sleeve!
